Award Nominations

These nominations are for EPIC coaches only.

The FCFSC Annual Banquet will take place on Saturday, March 4, 2017 at the Ptarmigan Country Club. The Leadership and Perseverance Awards will be given out at the banquet, and only coaches may nominate students for these awards.

Coach nominations are due by January 23, 2017. As coaches, you are the only people that can nominate skaters for these awards, so please take the time to do so, as these awards mean a lot to our skaters. You may only submit one nominee per award, and the skaters do not have to be your students. You must list 2-3 reasons why the skater is deserving of the award – these can be in the form of complete sentences or a numbered list. Skaters cannot win the same award twice; please do not nominate a past winner for the same award he/she has already won (but he/she may win the other award).

Submit nominations one of three ways:

1. Option 1: Fill out the digital forms below (just fill-in and hit submit)
2. Option 2: Email your nominations to Erika: erikaslehman@gmail.com
3. Option 3: Download this PDF form and place a hard copy in the locked file cabinet in the green rink (next to the coaches room door). You may fill it out by hand or attach a typed nomination.
